Analysis of 587 Mammograms taken by Soft Tissue Technique


Abstract
Since Leborgne reported the radiographic findings of breast carcinoma in 1951, the possibility of radiological diagnosis of breast diseases was studied by many authors. Egan was the first who reported very high accuracy in diagnosis of breast carcinoma and thereafter, mammogr-aphy was widely used for detection and accurate diagnosis of breast carcinomas.
Author experienced 587 cases of mammography using soft tissue technique during last 2 years from January 1975 to December 1976 in Cancer Research Hospital and among them 132 cases were confirmed by pathological study. Radiographic findings and clinical features were analyzed.
The results are as follows:
1. Of 132 cases. there were 62 cases of breast carcinoma and remainder were benign.
2. Diagnostic accuracy was 79.0% for malignancies and 60.0% for benign lesions, being 68.9% in total.
3. Carcinoma was most frequent in the age group of 30¢¥s and 40¢¥s, 77.5% was included in this group.
4. Most frequent clinical finding of carcinoma was palpable mass, present in 77.4% and pain and ¢¥tenderness was the next, present in 19.4%. 2 patient had no symptom and were regarded as occult carcinoma.
5. On mammography, mass¢¥ was present in 74.2% and secondary signs were present in high percentage. Calcification was present in only 17.7%, and this is considered due to technical error.
6. Diagnostic accuracy was higher in cancers of large size more than 2cm, being 85. 1% compared to 50.0% of that of small size of less than 1cm.
7. Diagnostic accuracy was also much higher in old age group over 40 years of age, being 87.2%, compared to 79. 0% of total.
It is assured that mammography is a highly accurate diagnostic method for breast diseases, especially carcinomas and great value lies in that occult cancers can be diagnosed before symptoms appear and still is impalpable. Thus, mammography is valuable for accurate preope-rative diagnosis and early detection of breast carcinomas.
